Stream Alteration Application Filing and Payment
INSTRUCTIONS:
  1. Download, complete and sign the Joint Permit Application Form, and combine the signed form with any required supporting documents into a single PDF document.
  2. Please fill in the form below with the requested contact information.
  3. Upload your PDF file that includes the Joint Permit Application Form and any required supporting documents.  The upload file size is limited to 25 MB.  If your PDF
    file exceeds this limit, you may upload a PDF file of the Joint Permit Application Form alone. You must then contact the Stream Alteration Program at 801-538-7414
    to make arrangements to supply any additional required documents. When contacting the Stream Alteration Program reference the order number or receipt
    number that will be given after payment is confirmed.
  4. Click to the Continue to Payment and you will be redirected to a third-party payment site.
  5. After submitting your payment, you will receive an email confirmation of the payment, and you will be redirected back to this website where you may view/print a receipt.
