REQUEST TO CONSOLIDATE WATER RIGHTS FORM
INSTRUCTIONS:
1. Please fill in the information below with the requested justification for this Consolidation of Water Rights.

3. All Owners/Applicants MUST then sign the form where indicated. If the owner/applicant is a corporation or other organization, the signature must be in the name of such corporation or organization by its proper officer, or in the name of the partnership by one of the partners, and the names of the other partners shall be listed. If there is more than one owner/applicant, ALL must sign, unless, however, a power of attorney authorizing one to act for all, accompanies the application.
4. Finally, Mail the Form or bring it to the Division of Water Rights ALONG WITH A FEE OF $50 FOR EACH CONSOLIDATION.
5. IMPORTANT: Water rights or application to be consolidated must be similar enough in all respects to be combined without creating difficulty in interpreting or distributing the combined water right. THIS MEANS THAT THE WATER RIGHTS TO BE COMBINED MUST HAVE THE SAME OWNERS, PRIORITY DATE, POINTS OF DIVERSION AND STATUS!!!
